Genital warts, sometimes referred to as condyloma acuminata, are flesh-colored or gray growths found in the genital area and anal region of both men and women. They represent the most common sexually-transmitted disease caused by a virus. The warts are caused by the human papillomavirus (HPV). Infection with genital warts may not be obvious.
Genital warts are caused by the human papillomavirus (HPV). Over 100 types of HPVs have been identified; about 40 of these types have the potential to infect the genital area.

* About 90% of genital warts are caused by two specific types of the virus (HPV-6 and -11), and these HPV types are considered “low risk,” having a low cancer-causing potential. Other HPV types have been strongly associated with premalignant changes and cervical cancers in women. HPV-16 is responsible for about 50% of cervical cancers, and types 16, 18, 31, and 45 together account for 80% of cancers.

* Common warts are not the same as genital warts and are caused by different HPV types that infect the skin.

Condylomata acuminata are anogenital warts caused by the human papillomavirus (HPV).

HPV is one of the most commonly transmitted STDs in the world. In 90% of women who have cervical HPV infection, the infection becomes undetectable within two years. A few women have persistent infection, which is a key risk factor for cervical cancer. Regular Pap smears and HPV screening can monitor this risk and provide an early warning that treatment is needed. HPV can be transmitted by vaginal, oral and anal sex. Many individuals who have HPV are not aware of the infection and can spread it to their sexual partners.

HPV types 6 and 11 typically cause venereal warts, and along with types 42, 43, and 44 have a low risk of progressing to cancer. HPV types 16, 18, 31, 33, and 36 have a higher risk of progressing to cancer.

These and other types commonly infect the cervix where they can cause either harmless changes or dysplasia which is a precancerous change. High-risk HPV infections of the cervix that last for years can result in cervical cancer. HPV infection of the cervix is found by Pap smear or cervical HPV DNA testing. A Pap test showing dysplasia or intraepithelial neoplasia or cervical cancer almost always is a result of HPV infection.

Approach to therapy

There are many self applied and office based therapies for warts. None of these eliminates the virus from surrounding skin therefore recurrences are possible. All therapies can cause pain, burning and erosions.

Wart often respond slowly to therapy and it may take weeks or months to achieve clearance. Home therapy is indicated for all clinical types of warts unless lesions are large or numerous. Acuminate warts respond more readily than papular lesions. Few, small lesions can be quickly removed in the office by electrocautery, scissor excision, cryotherapy or trichloroacetic acid. Removal of large lesions or clusters of lesions may have to be removed in the operating room. Change the method of treatment if 2 or 3 sessions with one modality fails. See the patient on a regular basis until all visible lesions have been eliminated.

Sex during the treatment could be uncomfortable and may have to be delayed.

Warts may regress spontaneously in some patients, therefore no treatment is an option.
